  BOOK NOW FOR MENTAL HEALTH - CASE MANAGER

The sports medicine team provides home coverage of varsity sports to all student-athletes to ensure the safety of the athletes.
The Sports Medicine clinic offers rehabilitation and treatment, injury assessment, injury prevention instruction.

The booking system will require you to provide an email address
and phone number in order to complete the booking process.


Student-Athlete Medical 
All Student-athletes must undergo a physical exam and be cleared to participate by a physician. This must be arranged and paid for by the individual student-athlete; Simon Fraser University does not organize this for you.

The medical, medical release forms and insurance information must be completed and on file in Sports Medicine prior to August 1st of that year.
 
The NCAA also mandates all student-athletes have knowledge of their sickle cell trait status.
 
All these forms are completed online by creating a profile on PRIVIT. Follow the link provided below in the step by step instructions.

Steps to complete the pre-participation medical:
  1. Follow the link: sfu.privitprofile.ca/index.jspa
  2. Register
  3. Complete all required sections of your profile
  4. Upload your insurance information
  5. From the Home page, scroll down to the blue "Print Documents" tab (your personal details & E-PPE questionnaire must both indicate 100% complete in order to access). Within "Print Documents" check the boxes beside the "Privit Profile Medical History Summary" and "Medical Examination Form" to print both forms and bring them to your physician to be completed.
  6. Upload your signed physical form from your doctor.
  7. Once your submission is complete, your medical will be reviewed. Check back daily to see if you've been cleared. Sports Medicine will contact you if there are any problems related to clearance.

Concussion Management Guidelines| Guidelines
SFU Athletics takes concussions very seriously. With the help of our Team Physician, we have guidelines in place to help athletes safely return to sport following a concussion. SFU's concussion management protocol includes; a baseline concussion test completed prior to the start of training and a Return to Play (RTP) progression following the SCAT 5 protocol for RTP. As concussions differ each time, and are unique to each individual and in symptoms, please use this information as a guideline and not an exact method.

 
Alcohol & Drug Use Policy
Simon Fraser University, in conjunction with the NCAA provides and supports both institutional and national testing. NCAA testing will be throughout the year, at their discretion, and arrangements will be made with a SFU Sports Medicine staff member, who will also the designated as the site coordinator. Please visit
the SFU Student-Athlete Handbook or our institutional policy.

Student-athletes may refer to the Resource Exchange Center, through National Drug Free Sport, for free information about dietary supplements containing possible NCAA banned substances.
